Patellar tendinopathy is overload of the tendon that connects the kneecap to the shin bone, often called jumper's knee. I focus on how the tendon is loaded day to day, because progressive strength work and smart training redesign are what usually change capacity, not a single quiet week of rest.
Pain is usually pinpoint at the bottom of the kneecap where the tendon starts, and athletes can often put one finger on the exact spot.
Jumping, cutting, downhill running, deep squats, and the day after a hard session are common flares. Morning stiffness that warms up with movement is frequent.
Unlike diffuse kneecap aching from tracking issues, this pattern is more focal and load-dose dependent, though both can coexist in the same active person.
The tendon adapts poorly to sudden spikes in jumping or heavy knee-extension load, and the tissue shifts toward a painful, less load-tolerant state rather than a simple overnight inflammation for many chronic cases.
Landing mechanics, quadriceps capacity, ankle stiffness, and sport calendar density all matter. The tendon needs progressive tensile load to rebuild tolerance.
True tendon rupture is a different emergency. What we usually see in clinic is intact tendon with focal irritability and reduced capacity for high-speed or high-volume load.
I palpate the inferior pole and proximal tendon, watch single-leg decline squat or hop pain behavior, and review training volume, surfaces, and recent spikes.
We separate this from patellofemoral pain, fat-pad irritation, and Osgood-Schlatter history in younger patients so the loading plan targets the right tissue.
Ultrasound or MRI is used when the diagnosis is unclear, symptoms are stubborn, or we are planning a procedure and need soft-tissue detail.
Progressive tendon loading through physical therapy is first-line, often with heavy-slow or isometric phases before return to jumping volume. A patellar tendon strap can help selected people during sport.
We redesign training so capacity rises without constant re-flares. Image-guided options are considered for persistent cases; corticosteroid into the tendon substance itself is generally avoided because of tendon integrity concerns.
PRP may be discussed for selected stubborn tendinopathy after a real loading program. Surgical referral is uncommon and reserved for refractory cases after thorough nonoperative care.
Not exactly. Patellar tendinopathy is focal tendon overload, usually at the inferior pole. Patellofemoral pain is often more diffuse around or behind the kneecap with stairs and prolonged sitting. Overlap happens, so exam detail matters.
Sometimes briefly during an irritable flare, then we rebuild jumping volume in a graded way. Complete long-term rest often feels better short term and then fails when sport returns without capacity work.
Injecting corticosteroid into the patellar tendon itself is generally avoided because of concerns about tendon weakening. We discuss safer options and peri-tendinous strategies when an injection is even on the table.
Tendons remodel on a weeks-to-months timeline. Consistency with loading and training redesign usually matters more than finding a single shortcut.
