Our team will confirm the service, clinician, location requirements, and next steps before scheduling.

Tell the team the general reason for your inquiry. We will confirm the available appointment options and whether you need an in-person visit. Please do not send medical records or detailed health information through the general inquiry form.

The Westport, Connecticut office is planned to open July 1, 2027. Telehealth is available now; specific services and clinician availability are confirmed before scheduling.

Virtual visits and in-person care

A video visit does not replace every physical examination. Injections and other procedures require an in-person setting. Adam Nessim, MD is licensed in New York, Connecticut, and Florida. Fernando Martinez, MD is licensed in New York and Connecticut.
